State the amount requested, what it will be used for, when it is needed, and what business result it is expected to produce.

Questions to answer before submission
What changed? What is the repayment source? Are there overdrafts, negative days, tax obligations, liens, merchant advances, or reporting gaps that must be explained? What happens if the requested capital is delayed?
